New to our office, here’s what you can expect at your first appointment:

Your initial appointment will consist of a consultation and diagnosis; often we will review treatment options at this visit, occasionally additional information and planning is needed and a follow up appointment may be necessary to determine the best treatment option for you.

Our new patient forms are provided below; please complete them prior to your appointment, this will save you time waiting in our office and will give you time to carefully review our policies and procedures. If you have any questions prior to your appointment please do not hesitate to call or email the office; we are here to serve you.

In an addition to the new patient forms (provided below) please provide the following information at or prior to your consultation.

• Your referral slip and x-rays, for most patients a current full mouth set of x-rays is necessary. If you do not have current x-rays we may need to take them during your appointment.
• A list of medications you are currently taking.
• If you have dental insurance, please bring all applicable information. This allows us to verify benefits and process claims on your behalf.
